STEP 1 – CALL TO SCHEDULE YOUR TELEVISIT
Please call (502) 995-5525. Our staff will help schedule your telemedicine visit with one of our physicians.
STEP 2 – INTERNET AND TECHNOLOGY
Prior to your televisit, confirm you have a working smartphone, tablet, laptop, or desktop computer with a functioning camera, microphone, and speaker. You will need an adequate internet connection.
STEP 3 – FORMS NEEDED
You will be receiving an email and/or text the morning of your appointment. Please open this link and complete in full the pre-visit interview. This needs to be completed prior to your appointment and in order to receive the link for your visit.
STEP 4 – ENTER THE VIRTUAL WAITING ROOM
15 minutes prior to your appointment you will be sent a link. Click on the link below at least 5 minutes before your scheduled appointment to be placed in to the virtual waiting room. Once your physician is ready to meet with you, your televisit will start
